Calvary Hill Health Services LLC offers a range of services that will meet the unique needs of your patient or loved one at home. We can design an individualized care plan for each client to include one or a combination of the following services:

Skilled Nursing
Acute Care, Medication Management and Symptom Control provided by Registered Nurses and Licensed Practical Nurses.

Personal Assistant Services (PAS)
Assistance with activities of daily living provided by a Personal Assistant.

Home Health Aide Services
Meal Preparation, Medication Reminders and Mobility Support for the elderly and post-surgery patients.

Physical Therapy, Occupational Therapy and Speech Therapy
A set of rehabilitative care services that are conducted by licensed therapists; the treatment will focus on recuperation for self-care skills, musculoskeletal skills and communication skills.

Medical Social Services
Administrative support by a medical social worker with services related to health management.

Primary Home Care/Family Care/Community Attendant Services (PHC/FC/CAS)
In the client’s home environment, our staff will provide patients with companionship, light housekeeping assistance and respite for family caregivers.

Medically Dependent Children Program/Home and Community Services (MDCP/HCSS)
Services that are designed to support families in caring for children who are medically dependent, and to encourage de-institutionalization of children in nursing facilities.

Private Duty Care
One-on-one care that is provided by Registered Nurses and Home Health Aides.

Cardiac Care
Therapeutic interventions, post-heart surgery care, care for patients with heart failure tendencies.

Gastrostomy / Colostomy
Care for patients with ostomies/stoma and incontinence issues.

Diabetic Care
Insulin injections, diabetic wound care, meal planning and preparation.

Urinary Catheter Care
Care for patients with catheter, bathroom assistance and mobility support.

Decubitus Care
Home therapy and treatment for decubitus ulcers and wounds, preventing bed sores for bed-bound patients.

Children with Special Health Challenges (SCHCN)

Calvary Hill Health Services LLC partners with the Texas Medicaid & Healthcare Partnership (TMHP) to provide for medical treatment and equipment for eligible children with special needs from birth to 21 years of age and for people of any age with cystic fibrosis.

Our Clients can get medically necessary health-care benefits and support services such as respite care (care provided by others). Clients and families can also get services like case management, which can help them find other services they need.

Our team of Core Professionals focus on families and help parents by working together with them. We work hard to make sure that families receive services that honor and respect their cultural beliefs, traditions, and values.

Comprehensive Care Program

The Comprehensive Care Program or CCP under Texas Medicaid provides for all the eligible patients from ages 0 to 20 years to receive all services that are medically necessary to preserve health and well-being.

It gives the individual access to Early and Periodic Screening, Diagnosis, and Treatment or EPSDT and an array of related services such as:

  • Care Coordination
  • Private Duty Nursing Care
  • Medical Supplies and Equipment (DME)
  • Speech Therapy and Physical Therapy
  • Psychosocial Services / Psychiatric Evaluations

Patient Education and Training

Once in the home, our skilled nurses, therapist, and home health aides provide the necessary education and training to effectively implement the physician’s plan of treatment.
